Only a parent or guardian may fill out this form. Email and contact information MUST match
Infinite Campus records.
The Clark County School District (CCSD) requires that attendance notes be submitted within three (3) school days of the student's return from the absence.
All non-medical absences are limited to ten per school year.
If you are unable to complete this form,
your student can bring the absence note in person to the front office.
